What to Bring

Please assist us by providing the following information at the time of your evaluation:

  • Your surgical referral slip and any X-rays, if applicable (You can request these from your dentist or referring doctor.)
  • A list of medications you are presently taking.
  • If you have medical or dental insurance, bring the necessary completed forms and information to expedite your appointment.
  • Make sure you have filled out the appropriate patient forms, which can be found on our website here.

Please Note: All patients under 18 years of age must be accompanied by a parent or guardian at the evaluation visit.

IMPORTANT: YOU MUST TAKE YOUR NORMAL MEDICATIONS THE DAY OF SURGERY UNLESS OTHERWISE INSTRUCTED BY YOUR PHYSICIAN OR DR. ANDERSON.

What to Expect

When you visit our office, we will conduct a thorough examination, complete any necessary scans, and review your medical history with you. This will allow us to make the most effective treatment plan for you, including choosing the right anesthetic to fit your needs. Ultimately, our top priority is to provide you with the safest and most comfortable surgical experience possible. 

Our office in Granger serves a wide region of northern Indiana and southern Michigan and offers a wide variety of treatment, from dental implant placement and wisdom teeth removal to corrective jaw surgery and oral pathology treatment. Our team looks forward to caring for you at our state-of-the-art oral surgery facility.

For patients undergoing IV anesthesia for immediate surgical treatment, a preoperative evaluation and physical examination are encouraged. Please have nothing to eat or drink for eight hours before your surgery and review our pre-operative instructions in detail. You will also need a responsible adult to remain in the office during your procedure and drive you home afterward.

During your evaluation, please alert the office if you have any pre-existing medical condition (e.g., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (e.g., heart medications, aspirin, anticoagulant therapy, etc.).

If your dentist or physician has taken X-rays, please request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.

For your convenience, we accept Visa®, Mastercard®, Discover®, and American Express®. We also participate in CareCredit® to offer payment plan options. We strive to deliver the finest care at the most reasonable cost to our patients.

Payment is due at the time service is rendered unless other arrangements have been made in advance. If you have questions regarding your account, please contact us at (574) 277-6637. A simple phone call can often clear any misunderstandings.

Please remember you are fully responsible for all fees charged by this office, regardless of your insurance coverage.

We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office in Granger, IN, if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. We can sometimes make arrangements for a monthly payment plan, but that must be agreed upon and implemented prior to the actual procedure.
