Your initial appointment at our Granger, IN, office will consist of an evaluation and may involve immediate treatment for some conditions. Dr. Anderson will explain your exam and treatment options and answer any questions you may have.
Please assist us by providing the following information at the time of your evaluation:
- Your surgical referral slip and any X-rays, if applicable (You can request these from your dentist or referring doctor.)
- A list of medications you are presently taking
- If you have medical or dental insurance, bring the necessary completed forms and information to expedite your appointment.
- Make sure you have filled out the appropriate patient forms, which can be found on our website here.
IMPORTANT: YOU MUST TAKE YOUR NORMAL MEDICATIONS THE DAY OF SURGERY, UNLESS OTHERWISE INSTRUCTED BY YOUR PHYSICIAN OR DR. ANDERSON.
All patients under 18 years of age must be accompanied by a parent or guardian at the evaluation visit.
For patients undergoing IV anesthesia for immediate surgical treatment, a pre-operative evaluation and physical examination are encouraged. Please have nothing to eat or drink for eight hours before your surgery and review our pre-operative instructions in detail. You will also need a responsible adult to remain in the office during your procedure and drive you home afterward.
During your evaluation, please alert the office if you have any pre-existing medical condition (e.g., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (e.g., heart medications, aspirin, anticoagulant therapy, etc.).
X-Rays
If your dentist or physician has taken X-rays, please request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
